I am aware that Valeriya Life Medical Spa allows rescheduling the appointment up to 24 hours in advance and up to 3 times. I am aware that Valeriya Life requires to text or call to cancel or reschedule appointments. If I cancel or reschedule within 24 hours of the start of the scheduled appointment time will be subject to a charge of $25. Failure to notify a member of Valeriya Life Medical Spa for missed appointments (no call/no show) before the start of the scheduled appointment time will be charged $50.
